To sign up for a Sansum Clinic MyChart account, please choose one of the options below. The sign up process can take up to seven days. An activation code will either be emailed or mailed to you, depending on whether the information submitted matches the information on file.

If you would prefer to request an account over the phone or otherwise need assistance, please call the MyChart Help Desk at (805) 898-3333, Monday-Friday from 8:00 AM-5:00 PM.

FOR NEW ACCOUNTS ONLY. Please do not attempt to sign up if you already have a Sansum Clinic MyChart account. You will not receive a response if you already have a MyChart account in our system. For help with logging in to an existing MyChart account, please call the MyChart Help Desk or go to the MyChart login page and use the "Forgot Username?" and "Forgot Password?" links.

This process CANNOT be used to request proxy access to pediatric patients ages 0-11 years old. If you would like to request proxy access to your child's (age 0-11) record, please go to the Sansum Clinic location where your child receives care. No appointment is needed for proxy access requests. You may also contact the MyChart Help Desk.
